Position Description
The Direct Support Provider is responsible for providing all aspects of direct care and individualized treatment for each resident of an independent residence by maintaining a safe and healthy environment and meeting program participant’s documented and known needs throughout their day.
The Direct Support Provider must be available to work a flexible schedule to include evenings, weekends, overnights, and holidays as assigned. The Direct Support Provider reports directly to the Program Manager. He/she assures that services support the program participant’s individual plan and are provided safely with each participant’s health and well-being kept in mind. He/she administers and documents oral and topical medication in a safe manner, per physician’s orders and UCP Heartland policy. He/she must have or be able to acquire skills to safely and effectively use assistive devices such as lifts, wheelchairs, splints, braces, communication devices, and other assistive technology. The Direct Support Provider supervises no other employees. He/she is expected to complete and submit to the Director, in a timely fashion, requisitions for needed materials and supplies, and requests for facility maintenance.
Requirements of the Job:
High School diploma or high school equivalent (GED)
Continuous employment eligibility through the Family Care Safety Registry, (EDL).
One year experience, paid or unpaid, working with people with disabilities.
Effective written and oral communication skills.
Basic computer proficiency
Preferred:
- Some college course work.
- Two years of paid experience working with persons with disabilities.
- Experience providing services at a facility-based respite program.
- Experience working with people with significant physical and/or multiple disabilities.
