There are two types of Tribute Gifts – an Honor Gift and a Memorial Gift. Honor gifts are made in celebration of special events or as expressions of gratitude. Memorial gifts are made in remembrance of someone who has passed away.
How to Dedicate Your Tribute Gift
Please include the honoree’s name and mailing address, so that we may notify that person of your thoughtfulness. We will include your name and address, but we will not disclose the gift amount. Please let us know if you would prefer to remain anonymous.
Please include the name(s) and mailing address(es) of the family member(s) to whom you would like an acknowledgment sent, as well as their relationship to the deceased. Checks should be made out to UCP Heartland. When mailing a check or cash, please include your name, address and daytime phone number so that we may send you an acknowledgement letter as a receipt for tax purposes. Please let us know if you prefer your donation to remain anonymous.
