Through our Residential Services we help create a life of freedom, dignity, and possibility for those living with disabilities, ensuring your loved one has the support they need to thrive.

Program Goals

At UCP Heartland, we provide people living with disabilities the in-home, individual support they need to foster independent living, with a roommate in their own home or in an apartment of their choice.

Program Highlights

We know that as a parent or caregiver, watching your loved one grow in confidence and independence is important to you. It’s important to us too. Through our Residential Services, our dedicated care team provides your loved one with the skills and assistance they need to reach their full potential.

For those clients who have already progressed to living in their own home or apartment, we provide the following services:

Independent Supported Living (ISL)

Our ISL program provides 24-hour individualized support to help people with disabilities live independently with one to three roommates. All aspects of your loved one’s personal, medical, and social care are provided by our care team.

Independent Supported Living Assistance (ISLA)

Our ISLA program provides up to 10 hours of weekly support for people with disabilities living independently in their own homes. Residents build essential life skills in home management while also being supported in medical care, job support, and socialization.

This year has been a remarkable period of growth and positive change for our organization. Thanks to our dedicated efforts to provide the highest quality care and necessary skills for independent supported living, Missouri families continue to see firsthand the power of our program and its participants. In 2024 we:

  • Proudly served 29 residential clients across 15 different locations.
  • Assisted 6 existing clients in relocating to better, safer, and more affordable homes.
  • Welcomed six new clients into the UCPH family during the last quarter of FY24, expanding our reach and impact.

Looking ahead in 2025

  • We have plans to continue our mission of providing safe and supportive living environments by filling two current vacancies and establishing another home by the winter of 2025.
