Whether you’re just beginning your journey or navigating a new phase of life, our compassionate team is here to offer a step-by-step plan and a connections to services tailored to your unique situation. We stay connected with you through the process, ensuring your needs are met to your satisfaction.

Program Goals

The goal of our Services Navigation program is to provide individuals and families—from birth through senior years—with personalized support during life transitions by connecting them to the right resources, services, and tools to gain and maintain independence.

Program Achievements

Throughout 2024, our Service Navigation team has continued to make a significant impact in the lives of those we serve. We look forward to accomplishing even more in 2025 as highly effective disability support Missouri families can count on. Celebrate our successes with us.

  • Personalized Support: We connect individuals to agencies that assist with job placement and retention, promoting independence through employment.
  • Unmatched Guidance: We assisted in guided a family through the diagnostic process, empowering them to advocate for appropriate services at school and support at home.
  • A Commitment to Excellence: In the last fiscal year, 307 individuals and families were served by the UCP Family Navigation Team.
  • PEncouraging Independence: We proudly helped a client transition from homelessness to stable housing, offering dignity and support.

UCP Heartland and Unite Us

We are proud to partner with Unite Us, a community data exchange of health and social service providers. Partners in the network are connected through Unite Us’ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. The United Way of Greater St. Louis is a strategic partner of the network.

Independence Through Technology (ITT)

The Independence Through Technology (ITT) program supports individuals by providing adaptive equipment and assistive technology. These tools are designed to promote greater independence at home, at school, at work, and in the community. 

Assistive technology allows growth in independence and reduced reliance on caregivers as well as increased self-confidence, efficiency, and overall quality of life. Let us help you find the assitive device perfet for you that will empower you to fully participate in daily routines and community life.

Some of the eligible equipment includes (but are not limited to):

  • Motorized Feeding Devices
  • Ring Doorbells (for safety and awareness)
  • Voice-activated Smart Home Devices
  • Adaptive Utensils and Kitchen Tools
  • Visual and Audio Monitoring Devices
  • Weighted Vests (for sensory regulation)
